Hazel Cortez Custodio

May 10, 1966 — May 3, 2025

Hazel Cortez Custodio, born on May 10, 1966, in Guam, passed away peacefully on May 3, 2025. She was the eldest daughter of Oscar and Zenaida Custodio. She was a beacon of love and compassion throughout her life, exemplifying what it means to care for others. Hazel dedicated her career to serving as a certified home health aide, where she made significant impacts on the lives of her patients, often cherishing the poignant stories that they shared with her. She also took care of mom during her time off, setting up and going with her to doctor appointments.

A proud graduate of the Academy of Our Lady and John F. Kennedy high schools in Guam, Hazel carried with her the values of dedication and kindness that were instilled during her formative years. She lived her life with a heart open wide, always putting the needs of others before her own. Her gentle spirit and humorous laugh became a comforting presence to all those around her.

Hazel had a special bond with her family, especially her beloved niece, Emma, whom she affectionately referred to as her "little cupcake." The joy of their shared moments was palpable and reflected her unyielding love. She also found pleasure in the simple joys of life, whether it was cuddling with her dogs, Zoe and Snickers, enjoying a good book, watching crime investigation shows (like CSI), savoring coffee from Starbucks, spending time enjoying foods with her friends and family, and taking pictures of beautiful flowers she's seen on her exercise walks in nature with mom. Hazel loved going to concerts and watching movies, often bringing warmth to every gathering.

Her legacy of love extends through her mother, Zenaida, her sisters Bonnie and Daisy, her brother Jude, her brother-in-law Gregg Zimmerman, her cherished niece Emma "Cupcake" Zimmerman as well as Daisy's fiancé Marco Castillo and his daughter Molly. Hazel is preceded in death by her father, Oscar Villanueva Custodio, whose memory she carried closely in her heart.

Friends and family are invited to celebrate the life of Hazel during a visitation memorial service on May 24, 2025, from 2:30 PM to 6:30 PM at Forest Lawn - Cypress - Hope Chapel, located at 4471 Lincoln Avenue, Cypress, CA, 90630. Here, we will gather to share their fond memories and honor the legacy of a loving daughter, sister, and auntie, who devoted her life to loving, caring for, and uplifting others.
